WASATCH YOUTH LACROSSE

SUMMER CAMP WAIVER & RELEASE OF LIABILITY

By registering my child for the Wasatch Youth Lacrosse Summer Camp, I acknowledge and agree to the following:

ASSUMPTION OF RISK

I understand that participation in lacrosse and related athletic activities involves inherent risks, including but not limited to:

  • Falls and collisions

  • Contact with sticks, balls, goals, or other players

  • Physical exertion and fatigue

  • Weather-related conditions

  • Serious injury, illness, permanent disability, or death

I voluntarily allow my child to participate and knowingly assume all risks associated with participation.

RELEASE OF LIABILITY

On behalf of myself, my child, and our family, I release and hold harmless:

  • Wasatch Youth Lacrosse

  • Wasatch High School Lacrosse

  • Camp directors

  • Coaches and volunteers

  • Facility owners/operators

  • Sponsors and affiliated organizations

from any and all claims, demands, causes of action, damages, injuries, losses, or liabilities arising from participation in camp activities, except in cases of gross negligence or intentional misconduct.

MEDICAL AUTHORIZATION

I certify that my child is physically able to participate in athletic activities.

In the event of an injury or medical emergency, I authorize Wasatch Youth Lacrosse staff, coaches, or emergency personnel to obtain medical treatment for my child if I cannot be reached immediately.

I understand that I am financially responsible for any medical expenses incurred.

INSURANCE ACKNOWLEDGMENT

I understand that Wasatch Youth Lacrosse carries organizational insurance coverage; however, I acknowledge that this coverage may be limited and is secondary to my own personal health insurance.

CODE OF CONDUCT

I understand that players are expected to:

  • Show respect to coaches, teammates, officials, and staff

  • Practice safe sportsmanship

  • Follow camp instructions and rules

Wasatch Youth Lacrosse reserves the right to remove any participant for unsafe, disruptive, or inappropriate behavior without refund.

PHOTO & MEDIA RELEASE

I grant permission for Wasatch Youth Lacrosse to use photographs and video footage of my child taken during camp for promotional, social media, website, or marketing purposes without compensation.

ACKNOWLEDGMENT

By submitting registration, I acknowledge that I have carefully read and fully understand this Waiver & Release of Liability and agree to its terms voluntarily.
